Whangamata Medical Centre welcomes new patients to enrol with us.

WMC is a Cornerstone Accredited Practice and is part of the Midlands Health Network.

We are currently enrolling patients who currently live in the townships of Whangamatā, Whiritoa, Onemana and Opoutere.
New patients will be asked to complete a Patient Enrolment Form when they first come in or one can be downloaded here:
Click here for Patient Enrolment Form Please print out form, complete and return to WMC – either in person by dropping into reception or by emailing completed forms and ID requirements to enrolments@wmcentre.co.nz .
Once we have received the completed forms we will request your medical history from your previous doctor.
Please note:
A separate enrolment form needs to be filled out for each patient, including all children.
Photo identification will need to be provided for all persons over the age of 16.
Patients over the age of 16 need to sign individual Transfer of Notes request form.
Your first consultation will be an extended 30 minute appointment, preferably after your notes have been received, so the doctor can familiarise themselves with you.
Consultations are by 15 minute appointment, or longer if required (although you may be charged for the extra time).
Nurses Consultation (30 mins) $30.00
GP consultation (30mins) $50.00
PLEASE UNDERSTAND THAT:
Failure to provide all relevant documentation and information will result in you remaining as a casual patient (and charged accordingly) until the enrolment process is fully complete.
We do not run accounts, so payment is required at time of consultation. Please see here for information regarding fees.
To make an appointment, phone 07 865 8032 between 9am and 5pm, Monday to Friday
Please let the receptionist know if your appointment is urgent, or if it requires extra time eg., an insurance medical or driving licence.
If you are unable to keep your appointment, please notify us as soon as possible as appointments are in high demand.
The doctors and nurses will do their best to see you on time but please understand that sometimes emergencies occur and these will take precedence.
You will be informed if there is a delay.
