Your privacy is of paramount importance to us.

Please tell us if you wish to talk in private with the Nurse or Doctor.

Should you want any member/s of your family, trustee etc to have access to your medical records on your behalf, please request an “Authority to Divulge” form from the receptionist (or click here) or you may like to discuss this further with your doctor.

Would you like someone to stay with you during your appointment?

• All patients are entitled to have a chaperone with them for any appointment

• The chaperone can be a family member, friend or a member of staff

• If you want a member of staff to accompany you during the visit please let the receptionist know when you arrive for your appointment, or tell your doctor / nurse

You are entitled to support under Right 8 of the Code of Health & Disability Services Consumers’ Rights 1996.
